Breathless is the eleventh chapter of Act 2 in The Devil in Me and the twenty-fourth overall.

The first segment is a cutscene and has no gameplay, focusing on what happened with Erin after the events of Silver Ash. During the second segment, the player controls Jamie (Theatrical Cut) and Erin (Curator's Cut) as Jamie makes a vital decision of who survives in the suffocation trap. This chapter can be replaced by Surgery.

Silver Ash Institute, 1:24 A.M.

• If Erin hid in the cupboard in Silver Ash, she exits the cupboard before realizing she left her microphone inside. After picking it back up, she turns around to find Du'Met sitting down next to her. She screams and he chases her into the metal chamber, with the door shutting behind her.
• If Erin did nothing or hid under the desk in Silver Ash, she awakes in the cupboard and questions what is happening as Du'Met picks her up and leaves her microphone on the floor. She begs him to let her go as he carries her into the metal chamber and sets her down, closing the door.

1:31 A.M.

2nd Floor Corridors

Jamie, Kate, and Mark follow Erin's scream. Kate finds her first and walks into the adjacent room when she sees Erin through the window. The door closes behind her and she realizes she is trapped.

Metal Chamber

• If Erin was carried into the chamber, she wakes up confused.

Erin looks around, realizing that she is trapped. She sees Kate run past and enter the adjacent room, calling for her to stop, but Kate cannot hear her and the door closes behind her.

Mark and Jamie explore the room, finding both doors locked, as Kate and Erin beg for help. Suddenly, the air in each chamber begins to be sucked out. Mark notices a regulator switch in front of them and deduces that whoever they point it at will die. Erin can choose to either beg Jamie for help or say that there is no hope for them and she is fine with being killed. As time runs out, Jamie must then decide who to kill:

• If Jamie chooses to kill Kate, Erin is freed. Mark then panics while an increasingly weakened Kate says there's a hole in her window. Mark breaks down the window, rescuing Kate. Kate and Jamie's relationship is reduced to zero and Kate slaps Jamie as Jamie checks on her.
• If Jamie chooses to kill Erin or does nothing, Kate is freed. Erin begins coughing up blood as she dies.
  • If Erin heard the edited audio tape of Charlie in Silver Ash, she writes "IT'S CHARLIE" on the window in her blood before dying.
